O-Module is very easy to install:

Open the file config.php and replace: 

$myCompanyName = 'Blah blah company name'; with your company name like 
$myCompanyName = 'My Company';

$from_email = 'your_email@server.com'; with your email address

and...

select all form fields and options you want to appear in the form.

!!!!Please READ all instructions!!!!
The script may not working fine if you make mistakes.

You don't need to know nothing about PHP and modify nothing else.
After all modifications in the config.php just upload all files by FTP.

Please note that free email addresses like Yahoo or Hotmail can deliver the email in bulk folder.
